Financial Information:
MSU SPORTS MEDICINE participates with most insurance plans. Please feel free to call us if you have specific questions.
In many situations, prior authorization or other paperwork may be required for your visit with us. You are responsible for being sure that these have been obtained before your visit. If you are unsure about your insurance requirements, please call your insurance representative. For your convenience, we have included our Financial Policy in a PDF format for you to review and sign when you visit our office.Insurance:
We participate with many insurance carriers. Please check with your insurance company or primary care physician to see if a prior authorization is required. The following is a list of some of the insurances that our clinic participate with:
Traditional BCBS, BCBS Community Blue, BCBS PPO (Blue Preferred), Blue Choice, Physicians Health Plan (PHP), Sparrow Physicians Health Network (SPHN), PPOM, Mclaren Health Advantage, and some of the United Healthcare and Aetna policies.
Please remember that not all of our physicians participate with the same insurances.
Fees, Billing and Payment:
Fees for office visits and surgical interventions are determined by the complexity of the problem; they may also vary with the amount of time spent. Our fees are comparable to specialist fees in the community. Our billers will be happy to discuss fees with you at any time. We want you to understand our charges and be satisfied that they are reasonable and equitable.
